Preface

In view of the promising development for tourist industry nationally & internationally nowadays. Tainan City Government also would like to aggressively strives to promote the local tourist by means of the existing well-conserved, unique and abundant tourist resources, A “Tainan City Travel Card”, the variety of service rendered in medical, police, language, information service and insurance aspects, is created consequently.
Tainan City Government aims at setting up an image of safe tourist city through this comprehensive device.

Procedures of Card Production

1. To fill the application form based on the in-force admission ticket.
2. To take a photo at the venue visited.
3. To take a completed card after 20 minutes.

Note

Insurance Coverage:
The insured who obtains the completed card within 7 days will be indemnified in respect of the accidental dismemberment or death while traveling in the region of Tainan. The above-mentioned accident excludes the accidental risk arisen out of any disease.

Sum Insured: NT$1,000,000 for accidental death 5%-100% of NT$1,000,000 based on Accidental Benefit Scale for accidental dismemberment.

Exclusion:
The insurer shall not be liable under the policy in respect of:

1. Any liability of indemnity by willful or incited act committed by the insured.
2. Any liability of indemnity by criminal act committed by the insured.
3. Any liability of indemnity caused by intoxicated driving at the level exceeding the law stipulated by the Road Traffic Regulation.
4. Any liability of indemnity by war (whether war be declared or not), rebellion, warlike operation. However, the extended coverage on the separate insurance contract excludes.
5. Any liability of indemnity caused by craft either unlicensed by local government or in the non-passenger status. How ever, the extended coverage on the separate insurance contract excludes.
6. Any liability of indemnity caused by explosion, glow. Ionizing radiation or contamination by radioactivity from any nuclear fuel.
7. Any liability of indemnity whatsoever nature directly or indirectly caused by any terrorism.
8. Any liability of indemnity caused by or in connection with or arising from some exercises such as wrestling match, judo, karate, boxing etc, or any acrobatics performance or contests.
9. Any liability of indemnity caused by motor, motorcycle, bike racing or performance. However, the above activities invited by Tainan City Government are excluded.

Beneficiary's Designation or Change If there is a death indemnity occurred, the claim will be made to the heir apparent, Otherwise, the designated heir will inherit it accordingly.
